Roselyn Singh, Team Lead Integrated Model and Health Coach  - Health & Wellness Coach in a Healthcare Setting

Nomination: Roselyn Singh is currently serving as Health Coach at Local Doctors Sylvia Park and as a Team Lead for the WST Integrated Model, In her short time with Tāmaki Health she had demonstrated a number of achievements and professional highlights:

Rapid Career Progression: Within just one year at Tāmaki Health, Roselyn's journey has been nothing short of inspiring. Starting as an experienced Health Coach, she swiftly distinguished herself through her dedication and innovative approach to patient care. Recognising her exceptional leadership potential and clinical acumen, and experience as a Health Coach she was promoted to Health Coach Practice Support. Recently, she was entrusted with the pivotal role of Team Lead for the South/Central clinics, where her strategic vision and collaborative spirit have impacted the delivery of wellbeing services and team dynamics.

Impactful Contributions: Roselyn has made significant contributions to the professional development landscape at Tāmaki Health in the Wellness Support Team. She led the overhaul of policies and practice guidelines, ensuring adherence to best practices and optimising operational efficiency for Health Coaches. Her initiatives in training health coaches on the utilisation of online resources and developing a patient Management guide for Medtech Evolution have empowered her team to deliver enhanced patient care experiences with seamless digital integration. Rose has also connected in with her network to bring tailored professional development sessions on long term condition management for Health Coaches which is meaningful for their context and role.

Leadership and Innovation: Beyond administrative roles, Roselyn's leadership shines through her innovative projects. She conceptualised and executed comprehensive training modules on "Heart Health," equipping her peers with specialised skills crucial for managing targeted behavioural health change for cardiovascular conditions. Additionally, her initiative in spearheading the Tāmaki Health Diabetes pathway has expanded access to health coaching services, positively impacting the lives of over 3000 patients through personalised care plans and proactive management strategies across the Tāmaki Health network.

Commitment to Equity and Patient Engagement: Roselyn has demonstrated a commitment to health equity by actively engaging with diverse patient populations, particularly the South Asian community. Her culturally sensitive approach in conducting risk assessments and lifestyle modification consultations has bridged healthcare disparities and fostered trust within marginalised groups, thereby promoting inclusivity and accessibility in healthcare services.

Future-Focused Initiatives: Roselyn's forward-thinking mindset is exemplified by her ongoing pilot program with Clozapine patients. This initiative aims to enhance long-term condition management and facilitate smoother transitions of care post-discharge from secondary psychiatric services. By addressing the unique healthcare needs of this patient cohort, Roselyn is paving the way for improved patient outcomes and continuity of care.

Patient-Centered Care: Roselyn embodies patient-centred care in every aspect of her work. Her calm demeanour and compassionate approach resonate deeply with patients and colleagues alike. She is adept at fostering meaningful connections within the clinic environment and adeptly handles challenging queries and relationships. Feedback from patients consistently highlights Roselyn's dedication to providing comprehensive support and connecting them with additional resources as needed.
